Workers' compensation is the nation's oldest social insurance program: It was adopted in most states, including California, during the second decade of the 20th century. The workers' compensation system is based on a trade-off between employers and employees. Employees are entitled to receive prompt, effective medical treatment for on-the-job injuries or illnesses no matter who is at fault and, in return, are prevented from suing employers over those injuries.
California Law provides for the following benefits to eligible injured workers:
Medical care to cure the injury
Temporary Disability
Permanent Disability
Vocational Rehabilitation - OR -
Supplemental Job Displacement Benefits
Death Benefits
Predesignation of Personal Physician Form

Notice of Establishment of a Medical Provider Network
City of Burbank MPN (COBMPN)
The goal of the MPN is to provide employees of the City of Burbank who have an accepted Workers’ Compensation injury with a single portal to access prompt and effective medical care with physicians and providers who understand the complexities of the California Workers’ Compensation system.
What is a Medical Provider Network?
An MPN is a group of health care providers (physicians and other types of providers) set up by an insurer or self-insured employer and approved by the Division of Workers' Compensation's Administrative Director to treat workers injured on the job. Each MPN must include a mix of doctors specializing in work-related injuries and doctors with expertise in general areas of medicine. MPNs must meet access to care standards for common occupational injuries and work-related illnesses. Further, the regulations require MPN providers to use medical treatment guidelines adopted by the State Division of Workers’ Compensation.

At any point in time after the initial medical evaluation with Concentra, you may select a physician of your choice from within the MPN. For help finding a treating physician please contact the Medical Access Assistant (MAA):

The MAA is available Monday through Saturday from 7 a.m. to 8 p.m. Pacific time.
Any person who makes or causes to be made any knowingly false or fraudulent material statement or material representation for the purpose of obtaining workers’ compensation benefits or payments is guilty of a felony.
Employee fraud can include lying for the purposes of obtaining or increasing benefits. It can be as simple as claiming mileage reimbursement for more miles than are actually traveled, telling their department that they are attending medical appointments for workers’ compensation when they are not, failing to disclose prior injuries or awards for permanent disability, working for wages while receiving temporary disability benefits, or making false statements about the extent of their disability or limitations.
